Shirt Modifier

(replace yyyy with any 4-digit shirt item hex code e.g. Big Bro's Shirt: 11CD)

Accesory Modifier

(replace zzzz with any 4-digit mask item hex code e.g. Big Bro Stache: 1451)

Also you may want to try 121d8fec and 121d8e4 as I suspect this will be the hat (but that is a total guess so I suggest you make a save of your game or use a legally dumped ROM of your card)
