The RUUP gamesave address is screwed up. I'll fix it soon.
In the mean time you can use the USB Gecko approach if you like (it puts the data in the wrong place, but it works). Follow the tutorial #1 - when it says you should get a black handled axe, you won't. But when you type 'guitar' it will work fine from then on.
darn. I don't have a USB Gecko. I'll buy one as soon as I get money. Please fix the gamesave address as soon as possible, because I love the item tool and I really want to test it out.

Can someone make some Easter eggs? It'll be fun to hide those and have people look for them XD

Fixed for RUUP (EU/UK/OZ "LetsGoToTheCity") GameSave (slow) approach. Sorry for the sloppy testing.
Please re-download the GCT file. http://www.aibohack.com/wii/itemtool_cheatcodes.zip

For USB-Gecko owners, please use the existing patch_ruup.bin. A cleaner fix will come later.

    No.
QUOTE: The graphics changes you make will only be visible in your town.
NOTE: To show others your work in progress, use the game screenshot mechanism. Later they may become "OHDLC" for wider testing.

There are already the built-in "Bunny Egg" items (which turn into Bunny Foil or ???). 13 of the eggs, 12 of the foil. Not sure how these are used in the normal game.
BTW: Some of these item transitions appear to be hard coded in the game (ie. can't be changed in DLC AFAIK). For example the wearing down and ultimate breaking of an axe is done with normal items only.

Are the item files downloadable from somewhere?
(I'm trying to recolor the Turban and a few types of glasses into Emerl's head (from Sonic Battle)
I don't have USB Gecko, but it's still fun to make the items.
